Scoreboard sometimes shows phantom clients. #20

Sometimes when a client is in the process of connecting or has left the game, they will appear in the scoreboard.

Possibly fixed by clearing team of disconnected clients at map change: https://github.com/Raptor007/aq2-tng/commit/598aa6a28eb0b7ae7098927f8435c68789e76d08

I believe this is fixed; if not, I will re-open the issue.
