Page 1: Colour contrast of "Select yes if you like geese" text.
The document doesn't have a title. The title tag is empty.
The fieldset does not have a legend.
Page 2: There's not enough validation on the inputs.
Page 3: There's not enough validation on the inputs.
Page 4: There is a skipped header level (h2). There isn't enough space between the h3 tag and the dl tag. The h3 looks like it might be part of the dl tag.
