Auto-populate feedback form with the project name

It'd be great if volunteers could give feedback for every project, and not just those in beta. I've put together a new project feedback form:

Currently, the form has a lot of pages asking for the Course / Module / project, but after a discussion with @martinpeck today it looks like forms can be pre-populated with the name of the project. This would leave us with a simple 1-page form, and would also mean I wouldn't need to update the form every time I write a new project :)

So, Martin I guess this means we need to:

• make it so that every project has a link to the feedback form, not just those in beta;
• remove the current form pages asking for project details and replace with a 'project name' and maybe 'course' fields;
• auto-populate the 'project name' and maybe 'course' fields.
