scenario

as a first step of supporting bulk FHIR, ndjson support should be implemented

steps

[x] check additional setup for including explorer
[x] use :explorer for loading csv files, pass all the tests
[x] create ndjson files as csv files counterpart using :explorer
[x] add ndjson support with :explorer, add tests and make them pass

acceptance criteria

[x] ndjson files supported via web interface
[x] all tests passed
