False positive reporting page triggered on DeniedUrl location call #4

It would be a Php/Python/? script allowing false positive reporting from 
end-users, although the URL will still be denied.

In order to avoid spam/flooding it should include a CAPTCHA mechanism.

Initial code has been commited in contribs but will certainly require testing 
and user reviews.
