add new automated CI and build process for the repo

master branch requires pull requests
require tests, approval, and code review on pull requests

Branch restrictions for master are now in place to require pull requests and to require at least one review from the code owners group before a pull request can be merged.

We should also move the builds to GitHub Actions so we're not relying on AppVeyor or Travis. This is a big enough lift, that I'm going to create a new issue for it.
