Removed duplicated lesson at the end #114

Lesson 40 was repeated as lesson 42 which was quite confusing as lesson 42 was supposed to be about error handling.

Transition the clipped lines of code into a new issue. And please describe what the lesson should cover. Thanks for the PR.

I found that there already was an issue describing this: https://github.com/ReactiveX/learnrx/issues/108

I am not really one to say what the lesson should cover, but it should not be a duplicate of an older lesson. That was very confusing.
