Added the demographics questions #8

I think I've correctly added the remaining demographics questions, but I'm not sure if i need to do something else (like save the data or something)

I'll delete this branch (we don't really need to use separate branches unless you wanna go hardcore) so on GH desktop, do:

  1. Sync (always start by syncinc even if there's nothing to pull)
  2. Go to the main branch make the changes directly on the main
