Add per-age bracket hospitalization rates to model #172

Is your feature request related to a problem? Please describe. We're currently using a blanket hospitalization rate across all age of individuals in the population. This likely over-represents hospitalizations in some cases, due to age breakdowns of the incarcerated and staff populations.

Describe the solution you'd like We should adjust hospitalization rate based on population age, the same way we currently do mortality rate.

Additional context We can use the rates provided here for the moment, but an e-mail thread with our epi contacts might produce newer or more reliable numbers soon.
