For repository permissions, you need read and write access to administration and code, otherwise the template can't create a repo. In the earlier sections for setting up GitHub Oauth, it only mentions setting permissions to read-only
I now see the permissions later on in the tutorial. It may be a good idea to move it up to an earlier step. I got hung up on trying to troubleshoot it before seeing it in the documentation.
