Implement support for Discord pomelo name and phase out discriminators. #83

Thanks to the genius people at Discord, names do not display correctly in-game anymore. The name which is displayed in-game is now the pomelo handle, which is probably not desired.

It should be displaying the user's display name instead.

ive always used %d4f:nickname% which doesn't seem to be effected :)
