improved function based on a larger redcap csv (bridge2ai) #33

The changes in this PR have been made since our EAB meeting when Satra gave me the bridge2ai redcap csv to help get DSM-5_adult for the nimh-minimal protocol.

The bridge2ai redcap csv is huge (1083 rows), and many errors occurred during converting. I was testing everything locally, and this current version works well at this moment (I successfully converted the whole CSV without observable errors). there are still things that need to be improved/optimized.

it depends on when Evan needs to use it; we can either merge this PR this week or wait until I have more bugs spotted & fixed.

just fix another error in ui order, let's merge this one then I'll start a new thread of improving
