Account request approval failure #367

In the RCAMP gui, if you set an approved on time BEFORE approving the account (set approve on time -> save -> set status to Approved -> save), the account is never created. This process fails silently. You should only be able to set the approved on time if the status is still Approved.

Alternatively, if the status is Pending, the approved on time should always be empty/cleared on save.
