243 docs fixes #250

These are mainly doc changes as outlined in #243. I've also included changes that have been made to the docblocks for the wp_calculate_image_srcset filter in core.

Rebased to merge the 2nd and 4th commits into one (d6b1319). I plan on leaving the rest separate unless there are objections.
