Support for Collections #34

Adding the 4 folders for collection support in Carbon:

All artwork by @UDb23, I just resized the controller icon files to 598px width x 350px height so that the aspect ratio compared to the other controller icons would be the same, without needing to change the theme xml for these specific systems.

EDIT: also updated the include reference from the default theme tag to point to the actual file, as we were getting a lot of lvl0 errors for not finding the file.
