Clean up remnants from Pitahaya site #111

Remove .meta.json files and the theme/skin/ files which were used to render the content to which the .meta.json files belonged -- while this was all a Pitahaya site.

Yes, I believe all the *.meta.json files can be cleaned up. We can salvage their content from git history later anyway.

The related templates are nicely spotted. We may end up with a similar approach later to describe parent-child relationships between pages, but in their current form they can go if they are unused.
