Feature/utf8 patch #26

A much cleaner branch for the UTF8 code. Probably still some tabs burried in there but easier to review now at least. There were a lot of files with only whitespace changes.

https://github.com/RhostMUSH/trunk/pull/26/files looks good to me all in all. I've prepared the Rhost Master as v4.0.0 and tagged 3.9.5p5 and made a branch off it, in preparation for this.

Ash, you see any obvious issues with the combined commit in that link?
