Fix #1

This also reduces the number of command lines used (more packages are installed per function call) and installs the RxODE and nlmixr tests.

Thanks for the suggestion @billdenney and sorry it took embarrassingly long to work on this. I restructured the repo to include a development and production version based on your pull requests. I included the tests (--install-tests) but could not easily check all the results. Maybe you can have look if it works for you as expected? Finally I got rid of the pdf explaining everything and added it to the readme file
