Paper form Associated with #32 #41

We need to have a paper form version of both the 'simple' and 'advanced' forms for Beaver Dam Data Capture in #32.

Okay @webernick79, for #32

Here we have it in order of priority:

  1. Beaver Dam Complex Form - Basic
  2. Beaver Dam Form - Basic
  3. Beaver Dam Form - Advanced
  4. Beaver Dam Complex Form - Advanced
  5. Beaver Activity Form

Note, the boxes are checkboxes and the circles are for radio buttons in form types (pull downs are fine in lieu of radio buttons)
