BREAKING - Allow for custom names

This change breaks current configs!

Instead of the old config entities:

• moisture:sensor.foo
• temperature:sensor.bar

You can (and must) now configure the card in a more common way: entities:

• entity: sensor.foo type: moisture name: Waterlevel
• entity: sensor.bar type: temperature name: Heat
