Replaces the "Decouple Entrances" and "Mix Entrance Pools" options with a combobox for each entrance pool that allows this. Grottos are left unmixed if they would be mixed in the same pool as dungeon entrances, to avoid the blue warp bug. Shuffle special interior entrances is moved to its own checkbutton.
Shuffle Overworld Entrances is bugged for reasons I don't know, on Decouple and Insanity it fails to generate, and on Mix it seems to decouple the overworld entrances when it isn't supposed to.
