fix compiler errors
fix issue with loading (require('../')) appmetrics
fix issue with (ebcdic) garbage being emitted in output

@mattcolegate I pushed to the ibmruntimes instead; I've now opened https://github.com/RuntimeTools/appmetrics/pull/600 for the AUTHORS.md change and CLA acceptance.
