Wrong Linking of elements in HTML #36

Due to changes in the eclipse build, it is not possible to create references to included section(2)-ref elements and there contents, as the name is determined by the chapter which is the output container of those elements.

steps to reproduce: make a document with two disjoint chapter-refs, let one of the referenced chapters contain a section-ref. in the referenced section, declare an anchor. now make a reference to the declared anchor in the chapter which is not containing the section-ref. voila, wrong url is generated.

fixed by injecting the index into the generator.
