Webhook Extension: Table View #315

We need a WP table for viewing our subscriptions to Twitch and some quick options for managing them.

This will go in the extension and make use of the core functionality that manages subscription data and upkeep.

Cancelling this for now as the custom post "webhooks" will cover viewing and management
