Add `@entityref` to `<ref>` and `<ptr>` #462

Needed as a migration path for the following:

extptr/@entityref extref/@entityref title/@entityref archref/@entityref bibref/@entityref

For <ref> and <ptr> (there are no ext* elements), OK. But <title>, <archref>, and <bibref> are no longer linking elements. Shouldn't the migration path be [title | archref | bibref]/ptr[@entityref]?

@tcatapano I wasn't suggesting adding @entityref to title, archref, or bibref, just noting that we needed a migration path for those, hence the need for adding it to ref and ptr. I'd been thinking the migration path would be to ref/@entityref.
