ead3 minor fixes #72

Resolves the following issues:

49 (titles to tags)

50 (codse -> codes)

53 abbr's mayoccurwithin now includes rightsdeclaration

54 lastdatetimeverified for sources

55 mark attribute had double values instead of summary+values

56 availability in representation

69 stray bracket in indexentry

I'm deeply chagrined that I let this branch molder for so long - apologies!
