table of contents and ref #77

A few quality-of-life fixes:

The table of contents is now nicely formatted! PDF TOCs now have dotted lines across the page to corresponding page numbers, and the attribute/element entries are now in nice columns down the page. There are a lot of elems/attrs, so I used two columns in smaller sizes, which helps condense without impacting readability. HTML TOCs are now tables, seven columns across.

Note that deprecated elements/attributes in EAD3 do not appear in the TOC - I'm not sure if this is acceptable or not.

A fix to #64 in PDF - external links now work in PDF (and a fix to the CC link on the second page).
