Generate real HL7 in the synth generator. Parse real HL7 in reader

Use an ad hoc parser for the ORU^R01 messages as it's orders of magnitude quicker than HAPI.

Missing:

• Sampling rate, as it's not given in the real messages. Will be fixed as part of #45
