Difficulty Level / Dynamic Quizzes

The instructor interface needs the ability to specify a number of problems a student should need to complete at each difficulty level in order to advance to the next level.
Example:
- An instructor decides that each student will answer 6 problems at the Low difficulty level
- The web app will only allow a student to advance to the Medium difficulty level after correctly answering 6 Low level problems.

This wasn't completed for RC1 because the client prioritized courses and user-types. Also it was requested that the teacher could assign to specific students. This is what was completed for RC1.

Quizzes are now dynamic. Teacher can set how many questions a student must get right and then student is given option to bypass the others.
