Intermittent problems with flat tets #53

Hi, we are for some reason having intermittent problems with flat tets. I can take the same input and crop it a little differently and go from no problems to thousands or millions of ERROR bad tet messages.

I know that debugging this has been ongoing-- would sample data be helpful? Any thoughts on what the difference could be between problematic and OK datasets?

This seems to be resolved with the latest version.
