ExternalIndex doesn't remove entries when a node's ancestor is unpublished #75

Umbraco handles this based on its own Index implementation, it's not done within Umbraco as part of a service layer. This means that it needs to be manually handled by ExamineX too.

Fixed in 2.0.9, 3.0.6 and 4.1.0
