Make presets neater #1313

This improves the documentation of the structured settings. It also adds the feature of enabling DATABASE_URL 12-factor configuration as a default configuration option. Using this feature, one could theoretically configure a new implementation of formhub without editing any Python code whatever. (In practicality, you still have to set some values -- like time zone, etc.)
