CSVs don't match what's shown on Matching Tool UI #58

Not sure who to assign this to.

The UI right now shows ID numbers that go sequentially (1, 2, 3...), but they should be list-specific, meaning B001, B002, etc for Baseline List and F001, F002, etc for Facility List. The CSVs should match exactly what is shown in the UI—just those five columns. I attached a screenshot of what the CSVs look like now, with some extraneous columns.

From Brett: .csv printouts have too many columns that are of no value to the TA. In my view, this .csv should show what the matching tool shows (ID, Ward, Fac Name, Community, Fac Type) and if possible, the file name should have the LGA name rather than X_lga_id in the name since that X_lga_id is only known to us here in NY. I’d make this a priority issue.

Also related: The Matched Facilities List shouldn't show "[NAME](Facility List) -- (Baseline) [NAME]". It should show "[ID] [NAME] matches [ID] [NAME]". If the IDs clearly reflect which list the facility came from, you don't need to indicate that with words. You can keep "--" instead of "matches" if you want.
