Slight improvements to progress tracking pages #82

  1. make homepage re-direct to /progress/facility_list/lga
  2. remove "lga,state" text to the right of "NMIS/LGA Facility Matching Tool"
  3. replace "education" / "health" with "Facility List Progress" / "Baseline Progress"
  4. Add a little bit of space to the zone column. So things don't go into two lines.
