WIP: finish source material markdown conversion #3

This is a work-in-progress PR to finish the conversion to markdown, specifically a markdown flavor which renders properly on GitHub. Most of the work thus far has been converting the HTML tables into either a markdown pipe_table or some other formatted text; there have been other minor changes as well, e.g. footnote and link conversions. There are some minor CSS tweaks as well to help improve rendering of the source changes.

Originally this effort grew out of a desire to improve the readability of the unrendered source material, but in the process of doing this I realized that the fully converting to markdown improved the rendered output as well as the CSS styles appeared to behave as expected (some of the PDF table hacks were no longer necessary).
