Heroku fixes #183

This is a follow-up to #182. I missed a couple of things the first time around, and for some reason the production app isn't even responding. So:

[x] Make sure that the app works (turns out I needed to set NPM_CONFIG_PRODUCTION=false to skip pruning of dev dependencies)
[x] Hard-coded references to "Vercel" and deployment URLs
[x] URL replacement rules in markdown docs
[x] .gitignore
[x] .npmignore should get app.json and Procfile
