removes unused things
Introduces TensorIndexer and Tensor to abstract tensor data structures (not yet very pretty)
makes DistributedFullGrid not-own the data vector
introduces OwningDistributedFullGrid as DFG with owned vector

the current goal is to have the selalib (and other libraries) examples not copying the dfg data.
