Footer, Page 2 (PDF Only): Missing Copyright Information #9

In PDFs in which the TOC extends beyond one page, the footers on the additional pages are missing the SMPTE copyright information.

As a reminder, the page 1 footer should contain the SMPTE contact information. If relocating it, then the page 1 footer also needs the copyright information.
