[x] Add simple symptom fields: Bilateral cataracts, Unilateral cataracts, Pigmentary retinopathy, Purpuric rash, Microcephaly, Developmental delay, Splenomegaly, Meningoencephalitis, Radiolucent bone disease
[x] Add "Congenital heart disease" with type that needs to be defined: Patent ductus arteriosus (PDA), Peripheral pulmonary stenosis (PPS), Ventricular septal defect (VSD), Other heart defect (specify)
[x] Add "Within 24 hours of birth" as a conditional field to Jaundice
[x] Add additional health conditions: Down's syndrome, Congenital syphilis
[x] Add "Notifying clinic" field: Pediatric-Inpatient, Nursery, EPU, CHER, OPD, Eye, ENT, Cardiology, Other (specify)
[x] Make sure special fields are correctly displayed in read-only forms in app
