Add sample source to case creation

Based on recent developments, there is a need to add a sample source field to case creation. As requested by file users this tallies with the case investigation forms and will help for easy disaggregation of data during analysis.

Please see attached file for ease. This will be as follows

• Field = options -Source of Sample collection = option 1: Routine Suspected case investigation option 2: Active case search option 3: Targetted Screening option 4: Community surveillance

@chukwuemekauzoho This is similar to #1898 , Where do you wish that this should be added in sormas please? can you add a mockup? I have a question fro you to find out please? This variable is about source of sample collections, thus related to the sample and should be on the idsrB1 form not A1 right? There can be a case without a sample

It is not clear is the variable should be added to the case form or to the sample form. Paper forms can combined all in one some times but when we migrate in sormas, we have to think of the best position to add it. Please organize a call with the teams to refine this further Thanks @Chinedar @foyiri
