6 years ago That is correct. It's "date of visit or admission. When the patient was not admitted we still need the date of visit.

In hospitalization the question WHAT PATIENT ADMITTED AT THE HEALTH FACILITY if you pick NO it still ask for DATE OF VISIT OR ADMISION
