Add a bug report template #1528

This adds an issue template for bug reports to the repository. When users will open a new issue they will have the option between a bug report issue and a regular issue. If they open a bug report they will be asked for a bunch of additional information to make sure we do not need to ask them for additional information. I based this template on https://github.com/stevemao/github-issue-templates/blob/master/.github/ISSUE_TEMPLATE/BUG-REPORT.yml, a repository that provides templates to other projects. You can see (approximately), how it looks like in their repo: https://github.com/stevemao/github-issue-templates/issues/new/choose (note I modified the template and removed irrelevant fields for specfem and added one or two new ones).

Happy to modify the template if you prefer other fields or information.

The files changed do not require a buildbot test. The maintainers can commit these changes.

Travis errored out with The job exceeded the maximum time limit for jobs, and has been terminated. on one of the tests, which seems unrelated to the changes in this PR. Do you have a way to restart Travis builds to see if it was a one-time failure?
