I had one issue. On page 301 of the book there is a footnote, and for the life of me I can't find the 1 it goes with. Can anyone with better eyes spot it?

I also couldn't find the footnote reference on pg. 301. @nicholasjhorton Can you take a look at this? I had a similar issue in chapters 03-06. I wonder if it is a misprint and what we should do about it.

Otherwise, I fixed some of the typos (smudges on the book, "AMHEEST") and added some missing page headers.
