adding beginning of chapter page numbers to "history_subtitles" #46

At present, the code that extracts the history_subtitles doesn't include the pages which start a chapter.

We will need to include these in order to add page numbers to history_text: #41

This needs completion of #36 and will facilitate #23

I don't know who did this or when, but kudos for your efforts: it looks great!

if you can retrospectively add a pointer to a commit I'd be very interested in how you fixed this problem.

As surmised, @jpapagelis24 added in prefatory text for each chapter so that page numbers would automatically be ingested.
