Fleshed out the example to make it clearer that we distinguish between change types and where issue identifiers go.
Also removed the pointer to the templates to avoid confusion with them now being handled by doc-kit. Also, the templates won't have any more info on this than the Style Guide.
Once the doc-kit stuff for Smart Docs is live, I'll adjust the templates in there accordingly.
