remove public-cloud.adoc from PDF as it is excluded from HTML as well

This PR is more to start a discussion and gather some information.

We have two files about public cloud in the administration guide:

• public-cloud.adoc
• public-cloud-azure.adoc

Both are excluded from the navigation in HTML, but only 1 was excluded from the PDF. This PR exclude also the other from the PDF.

Question: Why are they excluded? Should we remove them?

It seems there was some restructuring work done, and public cloud chapter was moved and re-written in the 'installation' guide. See:

If all the content is available somewhere else, we can remove the files from admin guide.

So, this is my fault. What's in the Install Guide is correct, and the content in the Admin Guide can be deleted.

We must not forget to cherry-pick it from manager-4.0 (or whatever the name of the 4.0 branch will be in the end).
