No place in "Disc Information" window to put Disc Key or Disc ID despite !submissionInfo.txt specifically having fields for that in "Extra" section (under PVD)

Addition of fields in "Disc Information" window to add a disc key and Disc ID to entry.

Additional context
Wii U should only need the additional "Disc Key" field, while PS3 would need a "Disc Key" and "Disc ID" field.
