Develop a simple demo with end points See #56 for main Hospital task - this

user simulator - simple html/javascript page that would be in the client
- 1: Selecting health provider from dropdown
- [ ] populate dropdown from the server
- [ ] Populate "self-isolate' text from server
- [ ] Add the hidden "replaces' field
