Use visit-wise R Ratio calculation #287

Using example 2 with a participant filter, filter to ID=5090-02, and start the animation. Participant disappears at day 84 (even though values are found).

🤬The R Ratio filter was causing the problem ... need to recalibrate filter defaults to look across all visits.

R ratio calculation now uses the maximum r ratio across all visits. This is considerably more conservative than using the max ALP and max ALT values (as was the case in the previous implementation).
