Support custom application name, home tab HTML, and hex image. #692

e.g. specify in the app configuration the name of the app (safetyGraphics by default in mod_safetyGraphicsUI), a path to the HTML (currently hard-coded in mod_homeTab), and the image file (also hard-coded in mod_homeTab).
